AGREEMENT TO PAY CHARGE OF SERVICE
The franchiser, [Name of the franchiser], and the franchisee, [Name of the franchisee], as set forth in their franchise agreement on [Date of the agreement] and as witnessed by [Name of the witness], have agreed to the following additional terms and conditions:
Franchisee agrees to pay [Name of Franchiser] at [Address of Franchiser], the following percentage of gross sales to the franchiser for the following [Amount of time to be paid]. Payments shall commence in increments of [Increments of payments] for a period of [Period to pay]. This agreement will be in effect from [First date agreement is effective] until [Last date agreement is effective].
This does include [Agreement inclusions] however, does not include [Agreement exclusions].
